Examples of Records that should NOT be keyed
The following records should be classified as “Cover page, Section Header, etc” forms and should not be keyed.
Declaration/Petitions with partial covering of petition. The declaration also would not be keyed as it shows a page underneath it.

Images such as film leaders, trailers, camera operator cards, etc (images that don't contain record information), should be marked as Image with no data forms.

This form type is for all images that have information about someone applying for citizenship in the United States, whether the image is of a Declaration of Intention, Petition for Naturalization or Certificate of Citizenship.
Prefix
Key any titles before the name, such as "Dr" or "Rev" in the Prefix field.

Given Name
Key the first name and any middle names or initials.

Surname
Enter the surname here as it appears on the image, using the drop-down list to help you. Check the applicant name against the signature. If they are different, use the signature as the surname and the applicant name as an alias. Use the most complete name.

Suffix
Key all titles following the surname, such as "Jr" or "III."

Alias Prefix
Key any titles before the alias given name, such as "Mr" or Mrs".

Alias Given
On some records, a second name is given in parentheses or quotes. This name is usually a nickname or maiden name. Key the first name or initial and any middle names or initials in the Alias Given field. If you cannot distinguish a given name or surname, please input the entire name in this field.

Alias Surname
Key any Alias Surname which appears. Occasionally an alternate name may be recorded for a given individual. This name may be denoted by parentheses. All the rules for keying a primary name should also be followed for keying any alias name.

Key any titles following the alias surname such as "Jr" or "III".

Date of Birth Day
Enter the day from the date of birth. If the date of birth is recorded on the image, do not enter the person's age.

Enter the full name of the month from the date of birth, using the dropdown list to help you. If the date of birth is recorded on the image, do not enter an age for this person.

Enter the four-digit year from the date of birth. If the date of birth is recorded on the image, do not enter an age for this person.

Age
Key the age in years, as it appears on the image. Ages less than a year should be recorded in a fraction (3 months should be entered as 3/12); ages less than 1 month should be entered as 0. If the date of birth is recorded on the image, do not enter an age for this person.

Birth Country
Key only the Country of Birth, using the drop-down list to help you. Do not key any other jurisdiction such as City, County, or Region.
Separate multiple countries with a semicolon space (; ). This also includes countries that were once part of a larger empire (e.g. "Finland of the Empire of Russia" should be keyed as "Finland; Empire of Russia"). Great Britian and Ireland should be keyed as Great Britain; Ireland. In some cases, the record taker may have recorded an allegiance instead of a nationality, like "Queen of England." If you can determine the country or nationality, please record that, otherwise, please leave the field blank.
- If the person was born in the USA and only the state is showing, mark the country blank.
Date of Arrival Year
Enter the four digit year from the date of arrival.
